When You Leave

On the day of your departure from Greenwich Hospital, your physician, nurse, and/or case manager will discuss your release with you. Please feel free to ask questions regarding your dismissal instructions. Your comfort and recovery are very important to us. Make sure you have made arrangements for someone to drive you home.

Case Management and Discharge Planning
Today's healthcare is very complex and patients' needs vary greatly. Each patient has a case manager who is a member of a team of nurses and social workers who make sure each patient's needs are met in the hospital or at home following discharge. The Day You Leave
On the day of your discharge, please plan on leaving the hospital by 11:00am. Your departure will be easier if you can arrange to send some of your personal belongings home the day before. Your understanding and cooperation will help us prepare for the next patient who may be waiting for a room.

Prescription Medicines
Medications prescribed by your physician may be filled at the hospital pharmacy before 4pm prior to discharge. The person who will drive you home can facilitate the prescription purchase for you. Contact the Pharmacy at (203) 863-3111.
