Medical Records & Birth Certificates
To request medical records
Connecticut state law requires hospitals to retain medical records for 10 years. Records older than ten years may no longer be available at the hospital.
Here is how to obtain medical records:
Complete and sign this
authorization form so that we know exactly what information to release and to whom we can release it. If you cannot download the form, call 203-863-3345, and we will mail one to you.
Once completed, you may mail, fax or hand-deliver the authorization form:

You may deliver the authorization form in person to the Medical Records office during business hours, Monday-Friday, 8am-5pm. We are located on the hospital's main campus, on the ground floor of the Watson Pavilion, 5 Perryridge Road, Greenwich.
To request copies of X-rays or other radiological images
Please call 203-863-3037.
How long does it take to receive medical records?
It takes about 10 business days from the time we receive your authorized release. As soon as the records are available, we will mail them to the address you provide. Please bring a photo ID if you choose to pick up copies of your records at Greenwich Hospital.
To request birth certificates
Birth certificates are
not available through Greenwich Hospital. They can be obtained from the Greenwich Town Hall, Vital Statistics Division:
