On the day of your departure from Greenwich Hospital, your physician, nurse, and/or case manager will discuss your release with you. Please feel free to ask questions regarding your dismissal instructions. Your comfort and recovery are very important to us. Make sure you have made arrangements for someone to drive you home.
Case Management and Discharge Planning
Today's healthcare is very complex and patients' needs vary greatly. Each patient has a case manager who is a member of a team of nurses and social workers who make sure each patient's needs are met in the hospital or at home following discharge.
