The Geriatric Health Team
Individuals scheduled for an assessment meet with our interdisciplinary team of professionals led by a geriatrician. Each team member contributes to your care. They include:
Geriatrician
This physician, board certified in geriatric medicine, specializes in the care of older adults. The geriatrician has particular expertise in the management of problems common to the elderly. Your own physician manages your ongoing care
and may request a consultation with a geriatrician for help in establishing a diagnosis and developing an appropriate treatment plan.
Geriatric Nurse Practitioner
A geriatric nurse practitioner is a registered nurse with advanced study and expertise in the assessment and care of older adults. The nurse meets with both patients and caregivers to identify potential problems associated with cognition, function and behavior. In addition, this specialized nurse works with the family and the health care team to develop a care plan.
Pharmacist
Many older adults take multiple medications – both prescription and over-the-counter – and have questions about their appropriate use and possible interactions. The pharmacist is available to review all medications and discuss their proper usage.
Gerontologist
A gerontologist has advanced knowledge and training in a broad range of aging issues. Patients and families contacting the Geriatric Health & Resource Center initially will be interviewed by the gerontologist. The gerontologist can help identify problems, assist with referrals and serve as an information resource for questions that arise.
Social Services Liaison
A social services liaison provides information about community resources and helps people access in-home assistance, respite care, housing, support groups, various entitlement programs and other services as needed.
